Community Support

Rainbow Center's psychiatric rehabilitation services focus on empowering the consumer to live a life of quality and dignity. Rainbow Center's Community Support program is an important part of this array of services. These services are designed to support the consumer, and in turn, decrease the number of hospitalizations the consumer experiences.

COMMUNITY SUPPORT GOALS

Rainbow Center believes that all consumers have something worthwhile to contribute to the program and to society. Through participation in this program, our consumers can take the steps to successful community living. Toward this end the Community Support program has set the following goals:

• Ensure the consumer access to services

• Provide for continuity and coordination among service options

• Provide services that are no more or less intrusive than necessary

• Ensure all interventions sustain a high level of quality

• Ensure that services are delivered efficiently and effectively

• Provide a clear focus of accountability for meeting the consumer's needs

• Provide checks and balances to ensure that decisions made are in the best interest of the consumer

ADMISSION CRITERIA

Community Support does not exclude persons solely on the basis of ability to pay, previous admission record, marital status, gender, race, color, national origin, religion, disability, or sexual preference. Community Support, provided by Rainbow Center, is for adults (must be at least 19 years of age). Consumers must have a diagnosis of severe and persistent mental illness, and must meet HHS System Management criteria.

THE SUCCESSFUL COMMUNITY SUPPORT CONSUMER

A "successful" Community Support consumer will no longer be institutionalized, will exhibit the skills, and be provided service necessary to live and participate as a member of the community. The consumer will live in a residence of his/her choice, and capabilities, and be empowered (have the authority, power and ability) to make informed choices to enhance self-responsibility.

CAN I USE COMMUNITY SUPPORT IF I'M NOT IN THE DAY REHAB PROGRAM?

Yes! Community Support is designed for those consumers that have never been a part of a day rehab program, but need the services Community Support provides. A hospital, HHS, a mental health clinic, clergy, a medical doctor, or the consumer themselves may refer to this service. Community Support is very individualized so consumers may use the service and may later become a part of another service provided by Rainbow Center. Some consumers may never use another Rainbow Center service.

Community Support is designed to service both pre, and post, day rehab consumers at the level that is most appropriate to each individual.

COST OF RECEIVING COMMUNITY SUPPORT SERVICES

There is no cost to consumers to receive services from Rainbow Center if the consumer is Medicaid eligible, or meets income criteria.
